Once successfully registering your child, please download the appropriate packet for additional emergency contact and health inventory forms.  Please complete and return to Y Packet completion is a required step of the enrollment process.

New Student Packet 
For Students that are new to the program or that cancelled care prior to the end of the 2007-2008 school year.

Returning Student Packet
For students that had continuous care through end of school year June 2008.

Change in Care Form 
please complete to make changes, additions, or to cancel your child's care.
