Become a Y Member

To become a Y member, you must sign-up in person at a family center Y location. Below are links to a few forms that may be needed in order to get your membership started. Forms vary depending on the type of membership plan you chose – a continuous membership plan or an annual membership plan.

  • Continuous Membership Plan – monthly payments are automatically drafted from your checking, savings, or credit card account on the 1st or 15th of every month.

    Depending on the day you join and the date of your first automatic bank draft, a GAP FEE will be due at the time you sign-up and may be greater or less than your regular monthly bank draft amount.

  • Annual Membership Plan – payment for a one-year Y membership is made up front at the time you join. Memberships are renewed on an annual basis.

Feel free to print and complete the forms you need, and just bring them with you when you’re ready to become a new Y member!

 

Membership Documents

Member Registration Form (required for all membership plans)
Provides us with basic information about you and your family; name, address, phone, email, date of birth, etc.

Also provides us with bank information needed to set up an automatic bank draft/EFT (electronic fund transfer). This is required for all continous membership plans. 

Financial Assistance Form 
Financial assistance is provided on a sliding scale to families and individuals in need. For questions about financial assistance, contact our billing and finance office at 443-332-8000.

Membership Change Form (for current Y members)
Acts as your written request to add or delete family members, change your membership type, change banking information, etc.

New Member Handbook (required for all membership plans)
Provides important information you need to know as a Y member. By becoming a Y member, you agree to read this handbook in its entirety and comply with all policies and procedures stated within.
 

Other Items

  • Driver's license/valid photo ID (required)  
  • Voided or cancelled check (required to set up an automatic bank draft using a checking account)
  • Bank Deposit Slip (required to set up an automatic bank draft using a savings account)
  • Credit Card (required to set up an automatic bank draft using a credit card account)
